If you haven't received your certificate, consider the following:

• Did you receive an invoice?
• Have you paid the invoice?
• Did you check your spam box in your email?

3. Where is my invoice?

Consider the following if you haven't received your invoice yet:

• Did you give your email address and/or fax to our office or the inspector so the invoice can be emailed to you?
• Did you check your spam box in your email?

4. How do I set up an inspection?

Fill out the online Boiler Inspection Request Form. You may also send an us email with the required information:

• Address of unit to be inspected (include city, state and zip code)
• Contact name
• Contact phone number

5. Where can I get a W-9?

It will be mailed with your invoice.

6. Who do I write the check out to?

Please write the check out to Office of the State Fire Marshal.

7. What is the process of getting a certificate?

The process is as follows:

• You are first inspected by a boiler inspector, either an insurance inspector or state inspector.
• The inspection is reviewed by the Chief Boiler Inspector.
• An invoice is generated with a deficiency notice (if have one).
• An invoice is emailed out.
• An invoice is paid and deficiency is corrected. All deficiency repairs/correction must have pictures and/or a repair invoice from the repairing company submitted.
• A certificate is generated and emailed to facility contact.
